These follicles remain immature and accumulate, giving the ovaries the characteristic "string of pearls" appearance often seen on ultrasounds. This lack of ovulation, or anovulation, is a hallmark of PCOS and contributes to symptoms such as irregular menstrual cycles and infertility.
